Unauthorized vehicle registration in Toronto - how to get ServiceOntario to remove it?

I recently discovered that a vehicle and Ontario plates had been registered in my name without my knowledge or authorization. I've tried to report the issue to ServiceOntario, but they just told me to submit a Record Search Application. I've also contacted the police, but they're not sure how they can help me. I'm looking for advice on how to get ServiceOntario to remove the vehicle/plates from my record.
