Starting in 2027, significant changes are coming to Medicare plans, and expats like me need to act now to ensure we're covered. As a remote worker based in Panama City since 2020, I've spent plenty of time worrying about healthcare.

Immediate Actions You Should Take

First things first, keep an eye on your mailbox in the coming weeks. By mid-September, you'll receive your Annual Notice of Change (ANOC), which could detail major changes to your current plan. If your plan is canceled, you'll have until October 2 to check for a non-renewal notice. Special Enrollment Periods are available, but you need to act fast. Make sure to mark your calendar for the Annual Enrollment Period (AEP) from October 15 to December 7, as all valid changes take effect on January 1, 2027.

Carrier-Specific Changes and Impacts

Three major carriers-United Healthcare, Humana, and Aetna & Anthem BCBS-are making significant changes. United Healthcare is dropping 13% of its plans in 13 states and is removing Worldwide Emergency Coverage from some plans. Humana is exiting 3 states and 194 counties, leaving expats without local alternatives. Aetna and Anthem BCBS are expected to cut benefits further, particularly in premium features.

These changes are driven by Star Ratings and performance metrics set by the Centers for Medicare & Medicaid Services (CMS). Highly rated plans receive substantial bonuses, while underperforming carriers are stripped of benefits to offset financial losses.
